the necessary premise for “health systems” to solve the problems presented above is the belief that the capability of organizations and their members to improve health is in fact an inherent property of the organization itself. we believe this position is empirically supported by the evidence that most health challenges are solvable. the central question is how to make the organization change the way it works so that the ability to solve the health problems follows automatically from that change. understanding the underlying causes of past and present failures can help answer this question.

how do we know that the lack of human and financial resources to support quality improvement (qi) activities is a persistent problem in health systems? this is a crucial question. if we know that qi is not a problem in most health systems, then why is it not a priority, and what are the mechanisms that lead to that outcome? what do we know about the opportunities and problems related to achieving “health care reform”? to address the concerns that policymakers have, the last component was a systematic review of published literature on research on and barriers to qi and we placed that evidence within a historical context to provide policymakers with some assurance that they are not starting a potentially costly project based on misconceptions. overall, the evidence is compelling that more research on qi in health systems is urgently needed. this review provides an important starting point for future work in this area.
