Postoperative bile leaks after gastric bypass for morbid obesity are a common cause of hospitalization and potential life-threatening sequelae. Endoscopic ultrasonography has been shown to be an accurate method of diagnosing bile leaks in the biliary tree. This study demonstrates the value of endoscopic ultrasonography in the diagnosis of bile leaks after gastric bypass. A retrospective review of consecutive patients with a history of gastric bypass surgery for morbid obesity who presented with symptoms of bile leaks between January 1, 1998 and January 1, 2000 was performed. All had bile cultures and all except 1 had had endoscopic retrograde cholangiography or laparoscopy. Endoscopic ultrasonography was performed with a 20 MHz endoscope and an 18-cm, 1-cm-thick, 5-MHz linear transducer. Endoscopic ultrasonography was performed only after negative bile cultures were obtained. If any free fluid was seen in the lesser sac, then patients were evaluated by the surgeon. If the surgeon found an obvious bile leak, then patients underwent either laparotomy or laparoscopy and were considered for laparotomy or laparoscopy only if the surgeon was unable to confirm a bile leak during the procedure. Twenty-six patients were included in the study. Bile culture was positive in 20 patients (77%). Endoscopic ultrasonography was performed in all 20 patients and bile leaks were confirmed in 14 (70%). The sensitivity and specificity of endoscopic ultrasonography were 70% and 100%, respectively. Nine patients had laparoscopy, 2 had laparotomy, and 4 were lost to follow-up.
